    UA Cossatot Benefits 

    UA Cossatot offers full-time employees a wide range of benefits.  Offerings include health, prescription, dental, vision, life, long term and short term disability, and critical illness insurances.  Full-time employees also receive paid vacation and sick leave, and an excellent retirment plan with employer contributions.  Employees may also receive educational assistance, in the form of free tuition, reduced tuition, or tuition reimbursements.  Dependents of full-time employees may receive as much as a 40% tuition waiver at any UA System school.

    MedImpact Prescription Drug Program:

    MedImpact is our prescription drug administrator for all three medical plans offered through UMR and United Health Care.

    The copays for covered prescription drugs under each medical plan at a retail pharmacy (up to a 30-day supply), is generally as follows:

    Health Savings Plan:

    • Tier 1:  10% after deductible
    • Tier 2:  10% after deductible
    • Tier 3:  10% after deductible

    Classic Plan:

    • Tier 1:  $15 copay
    • Tier 2:  $55 copay
    • Tier 3:  $90 copay

    Premier Plan:

    • Tier 1:  $10 copay
    • Tier 2:  $50 copay
    • Tier 3:  $80 copay

    Superior Vision Insurance:

    The College offers two vision plan options--The Basic Plan and the Enhanced Plan.  Both plans are administered by Superior Vision and pay benefits for eye examinations, prescription eyewear and contact lenses.

    You may see a provider in the Superior Vision National Network or an out-of-network provider.  However, if an out-of-network provider is used, the plan will pay less toward incurred charges.

    Standard Life Insurance:

    Policies Offered by Standard Insurance:

    • The College provides, as a benefit, Basic Life Insurance coverage that equals an employee's annual salary, up to $50,000 in earnings. 
    • If employees choose to elect, additional Optional Life Insurance is offered, at the employee's expense, in coverage amounts up to four times their annual salary, but not more than $500,000.
    • Dependent Life Insurance can be bought, at the employee's expense, for a spouse and/or eligible dependent children.
    • The College provides Long Term Disability Insurance (LTD), of 60% of an employee's pre-disability salary up to $20,000.  Employees have the option of buying addiitional LTD coverage at their own expense.
    • Short Term Disability Insurance is offered, at the employee's expense, of a maximum of 60% of the employee's pre-disabilty earnings, less Deductible Income. 
    • Accidental Death & Dismemberment Insurance is offered at the employee's expense, in units of $25,000 up to $300,000 for employees, with spouses being covered automatically at 60% of employee's elected coverage amount, and dependent children being covered automatically at 20% of employee's elected coverage amount.
