An individual with a “disability” is defined by the Americans with Disabilities Act (ADA) and Section 504 as a person who:
Has a physical or mental impairment which substantially limits a major life activity;
Has a record or history of such an impairment; or
Is regarded as having such impairment.
Documentation Requirements: Ask a professional in the field of your disability to provide information from an evaluation as indicated from the items on the checklist below. In general, the information should be from an evaluation done within the past three years.
